A. Holdback
- $2500.00
- $250.00 per statement for 10 statements
- paid interest twice a year
- paid back after 90 days as per Owner Operator Agreement

B. Financial
- decals and numbers at OK cost
- OK to pay first three months of insurance based on accident free performance
- OK to pay PRP portion of license
- statement of income plus back up
- OK pays all tolls
- $200.00 float in US funds for tolls

C. Group Benefits
- group package available through Maritime Life at Owner Operator cost to be deducted per statement

D. Fuel
- OK provides fuel cards
- NTS cards

E. Insurance
- 5 1/2% of gross revenue based on being accident free
- $2500.00 deductible on accidents
- invitation to Safety banquet after one year of service

F. Repairs
- two inspection per year at OK’s expense
- OK can repair at shop rate ($40.00/hour)
- PO’s provided and required
- Administration fee for work orders ($7.50)

G. License
- Owner Operator pays for Ontario plate only
- OK pays for PRP portion and all US permits
- OK to provide fuel tax processing service

H. W.C.B.
- OK pays

I. Seminars
- Safety meetings (twice yearly)
- Business meetings with management (three times per year)

J. Uniform Allowance
- if on specific contract - $250.00 per year
